If you did not go through the committee process, the deadline to submit the Health Professions Committee Application has now passed.  Students/alumni still planning to apply to health professions schools can now utilize the Credentials Service.  Click the "Forms" button for more info.





Recommendation Waiver Form
This is a great time of year to request letters of recommendation. To do so, please give the following form to your recommender. Note: Only 2010 applicants participating in the committee process need to have letters in by June 19, 2009.

Master List of Schools
You need to submit this form both in hard copy and electronically to the office plus a hard copy of your AMCAS, the Institutional Action Form, and the AMCAS Letter ID form in order for us to send out your committee letter and letters of recommendation.
