   - Health Professions Committee Application (HPCA)
   - Resume
   - Unofficial transcript (You can get this as a PDF through ISIS)

   Submit in Person to Office
   - Hard copy of HPCA
   - Transcript (unofficial is fine)
   - Hard copy of resume
   - Photo (no larger than 3" by 4")
   - $80 fee

?  Research Schools

?  Attend Small Group Applicant Meetings throughout the spring semester of 2009

?  Request Any Additional Letters of Recommendation - SUBMIT BY June 19, 2009

** Remember that every letter of recommendation MUST BE ACCOMPANIED BY a Recommendation Waiver Form.**

?  Begin your AMCAS/AACOMAS Application

?  Submit your AMCAS/AACOMAS Application - Aim to do this in June or July

?  Submit secondary applications - do this ASAP upon receiving them

?  Prepare for interviews
