Registering for the First Time with Student Disability Services

All admitted students with disabilities who wish to receive accommodations must initiate the registration process by contacting the Student Disability Services Office.  The SDS Office will ask you to complete these steps:

Step 1: Send Documentation
In order to make the most appropriate accommodations for a student with a disability, it is necessary to have professional documentation to establishe the student’s eligibility for services, explain how the disability limits essential life functions in the university setting, and recommend reasonable accommodations that will allow equal access to programs, services and activities at the university.  Documentation guidelines can be found at http://web.jhu.edu/disability/students/admitted/documentation/index.html.

Step 2: Complete the Intake Questionnaire
Print out and complete the Intake Questionnaire which can be found here.  Return the completed form as an email attachment to studentdisabilityservices@jhu.edu.

Step 3: Schedule an Interview with the Director
Your documentation and answers to the Intake Questionnaire will provide us with important information about you.  But a personal interview allows us an opportunity to go beyond the forms, to get to know you as a person, and to assess what accommodations will work best for you.  It is also an opportunity for you to ask questions or express concerns that you may not have wanted to raise previously.
